Dementia Assessment & Living Well Plan

Understand the person behind the diagnosis — and create a practical framework for living well.

Person first

This is about how someone can continue to live well.

Where clinically appropriate, we begin with a clinician-led assessment that considers the person’s abilities, interests, cognitive and functional needs, daily routines, strengths, goals, previous activities, relationships, community connections and family circumstances.

The assessment helps identify what the person can continue doing, what they enjoy, and where additional support could make a difference.

The outcome is a personalised Living Well Plan — a practical framework for ongoing WellKind support. Not simply a list of what someone can no longer do.

How the Living Well Plan is used

The plan guides Dementia Engagement Workers, family coaching, cognitive engagement and clinical review.

Guides engagement

Support is shaped around what matters to the person — hobbies, routines, community life and goals.

Supports the family

Families gain a clearer picture of how to encourage participation and independence at home.

Evolves over time

Clinical review adjusts the plan as needs, abilities and circumstances change.
