Adapt · What We Offer

Clinical Reviews

Support that changes as the person changes. Dementia is not static — and neither should support be.

Part of the specialist model

Review. Adjust. Continue living well.

Where clinically appropriate, WellKind can provide regular clinical review of the person’s program and adjust their Living Well Plan as needs, abilities and circumstances change.

Reviews may consider cognitive engagement, functional abilities, meaningful activities, community participation, family concerns, changing behaviours, goals, support strategies, and new risks or barriers.

Clinical services are provided by appropriately qualified professionals. Not all WellKind employees are clinicians.
